We are able to support participants with their high intensity support needs

We deliver high intensity daily personal activities for people, at home and in the community. Support staff receive generic training regarding the specific needs, and individualised training to ensure they are able to support these needs.

A support plan for high intensity supports is developed and overseen by a relevant health practitioner. Each participant is involved in the assessment, and development of their support plan.

The NDIS support plan is kept up-to-date, readily available, clear, concise, identifying and describing the support needs and preferences of the participant.

Participants are supported to seek regular and timely reviews of their health status, and support plan by an appropriately qualified health practitioner. Each participant’s support plan is communicated, where appropriate and with their consent, to their support network, other providers, and relevant government agencies.

We ensure that our ndis support coordinators understand the support needs outlined in the support plan.

This ensures they can assist with and/or supervise complex supports for people, who require high intensity daily personal activities, to meet their disability related health needs. This enables the participant to live as autonomously as possible. These supports are overseen by an appropriate health professional, and provided by a support worker.

These individual supports can be provided in a range of environments, including but not limited to the participant’s own home.
